 Aged Care, and specifically the need for in-home support, is experiencing unprecedented growth in demand. Our client is an innovative entrant developing a new marketplace for care plans across Australia. They have a vision for a world in which those who are getting on in years and still live at home have an enriched and fulfilling life tailored to their own circumstances, and more importantly, their desires. This requires that services are not isolated to just help or care elements but will include lifestyle choices and planning.  

Opportunities like this do not come along very often. This role is not for the average care plan designer, but rather a respected aged care coordinator who is excited about starting something new from the ground up. You will be asked to bring your ability and experience to innovate and do things differently; to guide and develop an improved solution in the provision of quality care to clients in their own home.

In your new roles, you will carry out comprehensive and accurate client assessments, and maintain appropriate documentation. You will design and implement tailored premium packages for individuals and providing ongoing customer service. You will be comfortable taking a leadership role in the coordination of plans, liaising with health care professionals and other suppliers to ensure compliance with relevant Industry standards and delivery of agreed services to a high standard. 

To be successful in this role you will need:

  • Relevant Home Care industry experience in case management of clients on home care packages.
  • Demonstrated knowledge of contemporary models of community aged care, particularly as they relate to Consumer Directed Care (CDC), and Client Rights and Responsibilities.
  • Demonstrated ability in the management of clients with complex care needs, especially in the development of care plans as a member of a multi-disciplinary team.
  • A certificate III in Aged Care or Individual Support.
  • Experience as Care Worker in Aged Care would be highly regarded.
  • Working knowledge of the Sydney and North shore market.
  • Tertiary Qualifications in Nursing, Allied Health or Social Sciences or Diploma qualification in Community Services.
  • Strong knowledge of CDC and HCP legislation.
  • Experience working with a care management software packages.
